Tethy-i Benedek egri vikárius megparancsolja a Kyswarada-i, Ramachahaza-i, Lewelek-i, Ibron-i, Apagh-i, Baktha-i, Bogach-i, Feyertho-i és Polch-i plébánosoknak, hogy miután Karlo-i Lőkös fia Jánost kiközösítette, mert felszólítása ellenére sem volt hajlandó megfizetni 40 aranyforint adósságát, hirdessék ki egyházaikban.- A szöveg alatt zöld pecséttel. - A hajtások mentén lyukas.
