A leleszi konvent jelenti Pálóczi László országbírónak, hogy Saralyan-i Illés királyi ember Szatmár megyében vizsgálatot tartott, majd Chegel(d) faluban megidézte Belthewk-i Dragh fia Miklóst Veronika, Belthewk-i Dragh fia Sándor öözvegye, Bwthka-i András felesége, ellen Vízkereszt nyolcadára a királyi jelenlét elé. A hajtások mentén kissé szakadt és lyukas. Hátoldalon rányomott pecsét csekély töredéke. LELESZI KONVENT
