A kalocsai káptalan jelenti II.Ulászló királynak, hogy parancsára Waryeth-i Gergfy Tamás királyi ember és Faad-i András kalocsai kanonok, hiteles ember július 15.-én vizsgálatot tartottak és ugyanazt állapították meg, amit a királyi parancslevél tartalmazott. - Hátlapján ovális zárópecsét töredéke. – Regeszta forrása: OL regeszta (Érszegi).
