Buda (f. VI. p. Exalt. Crucis) II. Lajos néhai Kaniych(a)cz-i Horwath Márk testvérei: Máté és András kérelmére, minthogy II. Ulászló adománylevele s a budai káptalannak ennek alapján kelt beiktató jelentése nincs kezeik között, Korlathkew-i Péter tárnokmester által kikerestette ”in libro sive registro” 1496. évben a 280. lapon a II. Ulászló 1496. június 10-én, György fejérvári Szent Miklós egyházi prépost relatiojára a királyi kettős pecsét alatt kelt confirmationalisát. Eredeti, hártya.
