A szepesi káptalan előtt Zenthywan-i (de comitatu Liptouiensi) Lampert fia: Péter eltiltja a maga, az apja, valamint testvére (fr. uterini): Benedek, továbbá ugyanottani Sebestyén fia: Miklós és Péter fia: János (de eadem) nevében is a királyt Lehotta nevű birtokuk eladományozásától, Lehotta-i Egyed fiát: Jánost, Barnabás fiát: Gergelyt és nővérük fiát: Miklóst pedig, akik a birtokot a királytól új adományul kérték, annak elfoglalásától. - Rongált papíron, hátlapján viaszpecsét darabkái.
