(f. IV. p. oct. epiph.) Szentmária (ad Beatam Virginem). Komorow-i Péter Lyptho megye ispánja és a szolgabírák bizonyítják, hogy Zenthiwan-i Miklós fia: István harmadmagával (met tertius se similibus nobilibus) letette Zemerechano-i Lőrinc fia: László ellenében az esküt, amely szerint ennek Nemethporuba-i jobbágyait nem akadályozta és nem akadályozza a Zenthiwan-i templomhoz és erdőhöz vezető utak használatában, nem okozott neki 20 aranyforintnyi kárt, s nem is hatalmaskodott; így felmenti őt. - Papíron, hátlapján egy nagyobb zöld és két kisebb barna viaszpecsét nyoma.
