Mátyás király megparancsolja Zala megyének, hogy a tihanyi apátságnak Apathy helységbe tartozó, név szerint felsorolt jobbágyait, akiket a veszprémi káptalan az ország törvényei ellenére Azofew possessióban lévő birtokrészére telepített át, telepítse vissza összes javaikkal együtt Apátiba. - Szakadozott, kissé hiányos. - Szöveg alatt piros viaszú, papírfelzetes rányomott pecsét.
