A Lelez-i Szent Keresztről nevezett konvent jelenti Ulászló királynak (regi Hungarie, Bohemie etc.), hogy az 1499 júl.1-i parancsában elrendelt tanuvallatást Zemplén megyében (Zempleniensis) Cheb-i István királyi ember és Félix áldozár, hites személy, megejtették és miután a vallomások a mandátumban írt panasz jogos voltát bizonyították, a hatalmaskodókat a panaszosok ellenében a kitűzött terminusra a király elé insinuatióval perbe idézték. - Hátlapján: pro A Thomas de Edeles cum Jazow non propria Barchay - Papír - Kitöredezett z.p. - Regeszta forrása: OL regeszta (Ila).
