Lastocz-i Simon Zemplénmegye (Zenpliniensis) alispánja és a szolgabírák jelenti Gwth-i Orzag Mihály nádornak (Hungarie) és a kunok (Comanorum) bírájának, hogy Kamonya-i János megyebeli szolgabíró 1465. szeptember 7.-i parancsában (290. sz.) elrendelt tanuvallatást a megyében megtartván, a kihallgatott tanúk vallomásaiból megállapította, hogy a mandátumban foglalt hatalmaskodás teljesen a panaszosok előadásának megfelelően történt meg. Papír, három zárópecsét helyével és egynek töredékével. ZEMPLÉN MEGYE
