A kapornaki konvent előtt Rayk-i Tamás tiltakozott az ellen, hogy Rayk-i Beke fia László egyedül vette fel Appay fia Istvántól azt a 100 arany forintot, amelyet ez a zalai konvent bizonyságlevelében foglalt egyezség értelmében neki lefizetett, holott az összeg fele - e kompozício tartalma szerint - őt illette volna. - Hátlapján rányomott pecsétmaradvány. - Több helyen kissé csonka, eleje vízfolt miatt olvashatatlan.
