A Lelez-i konvent bizonyítja, hogy Zapolya-i István szepesi (terre Scepusiensis) örökös ispán nádor (et iudicis Comanorum) Budán (Bude) Vízkereszt 20. napjának 17. napján /febr.10./ kelt ítéletlevele értelmében Peren-i Gábor Bethlenfalwa-i Thwrzo Teofil és Bozytha-i Cherubin ellenében a megítélt esküt 50-ed magával letette. - Hátlapján pecsét nyoma. - Regeszta forrása: OL regeszta (Fekete Nagy).
