sexto die f. Joh. Bapt. A leleszi konvent jelenti II. Lajos királynak, hogy 1522. április 23. körül kelt parancsára az ahhoz mellékelt, ugyan ezen napon kelt mandátumot a dímzetteknek: Bylkey Györgynek, Pereni Gábornak és társaiknak kikézbesítette és egyben a hites személy a mandátum tartalmát a megjelenésre vonatkozóan élő szóban is közölte velük. (Csonka szöveg.) Eredeti, papír, hatalmas hiányokkal, mintegy fele hiányzik, de e meglevő résznek is a fele olvashatatlan. Zárópecsét maradt.
