Kassa. in vig. Ascens. Domini. Zsigmond király megparancsolja a nádornak, az országbírónak és valamennyi bírónak, hogy Mindszent nyolcadára (november 8.) halasszák el Kallo-i Lőkös Miklós fia Jánosnak és név szerint felsorolt négy familiárisának minden perét, mivel az erdélyi püspök bandériumában katonai szolgálatot teljesítenek. Eredeti, papír, ragasztott, hiányos, elmosódott, a szöveg alatt papírfelzetes vörös pecséttel.
