Mekcsei György szepesi prépost nyugtatja Gimesi Forgách Pétert és Gergelyt, miután ezek neki Forgách Benedek özvegye Borbála meg Mekcsei Balázs özvegye Anna asszony végrendelete értelmében – Anna asszony végrendeletének a prépost executora volt – egy kis arany gyűrűt, gemmát és 12 vég vörös színű kamukát átadtak. – Szöveg alatt jobbról sk. aláírás. – Piros viaszú, papírfelzetes ép rányomott pecsét. – Regeszta forrása: OL regeszta (Ila).
