Buda. 8. d. [20. d. Georgii]. [Ujlaki Lőrinc országbíró] Erzsébet asszony: Pesthenye-i Saffar Tamás felesége kérésére átírja Ráskai Balázs királyi tárnokmester, a nádori presentia királyi kiküldött bírájának 1504. december 1-i (?) ítéletlevelét és a pozsegai káptalan 1504. január 14-i oklevelét. Eredeti, papír, kb. a fele (felső része) hiányzik. A szöveg alatt papírfelzetes pecsét. A csonka oklevél kibocsátóját a szöveg alá nyomott pecsétről lehetett megállapítani.
