A /nyitrai káptalan II.Ulászló királynak/. Amikor 1504. május 17-i parancsára Felsewlephanth más néven Kyswcza és Kyspodlusan /csonkaság miatt neveket nem lehet megállapítani/ be akarták vezetni, annak Felsewlephanth-i Thwroczy Benedek özvegye: Anna asszony a maga és fia Boldizsár nevében ellentmondott, amiért őket a királyi perszonális prézencia elé perbe idézték. – Három darabra szakadva, az elején egyharmada hiányzik, hiányos a szakadásoknál is. – Zárópecsét töredéke. – Regeszta forrása: OL regeszta (Ila).
