Anna királyné meghagyja More Eleknek, a diósgyőri vár castellanusának és utódaiknak, hogy ha megürül a várhoz tartozó Chaba possessió plébániája, amelyre a király Kelemen, miskolci papot eligálta s nominálta s egy másik oklevélben Anna királyné is eligálta és nominálta, őt annak élvezetébe iktassák be. - A szöveg alatt balra: commissio propria domine regine. - A lap alján, a jobb sarokban: Jo.
